How collective decision-making and practice, enabled by innovative commissioning approaches, can lead to clinically-led and community-owned service redesign and improvement. This session draws on the work of a partnership in north east London that is improving access, experience and outcomes for black and Asian people with cardio-renal-metabolic disease.
