This case study will look at how GPs in Warrington grew from a single surgery to a primary care network, and with the support of the integrated care board are set to grow across the whole of Warrington. For patients with chronic obstructive pulmonary disease and those with hypertension, the team offers app-based oxygen saturation and blood pressure tracking at home. A population health management approach identifies those at highest risk of health issues and supports appropriate action.
