28 per cent of the population in England identify as disabled.

This session puts into context the health inequalities faced by adults with a learning disability and autistic people, a key priority in NHS England’s Core20PLUS5 approach, and how reasonable adjustments can help to address them.

It will include the steps Wyre Forest Health Partnership has taken to create a local digital flag/alert and use case studies to show the positive impact this has made on the patient journey and patient outcomes.
