The urgent and emergency Care (UEC) recovery plan estimated up to 20 per cent of emergency admissions can be avoided with the right care in place. It committed to offering more joined-up care to help people get the care they need without requiring a hospital admission.

This session will explore how integrated care coordination enables clinical conversations to ensure patients are referred to the right care in the right place first time and improves patient experience.
