This session will explore how we can make every interaction count in primary and secondary care while building community capacity to improve care, reduce inequalities and help stop or delay disease. We will explore evidence-based models and good practice in secondary prevention across our health and care system, and consider how we can address gaps where more needs to be done, such as in maternal health and mental health.
