Transforming care pathways
The session will focus on the NHS Race and Health Observatory’s work on identifying digital care plans and pathways for healthcare leaders and sickle cell patients.
Panellists will discuss the findings of a digital discovery project undertaken in collaboration with nursing, healthcare professionals, red-cell specialists and the lived experience of people living with sickle cell. The session will be led by Dr Carl Reynolds, who will discuss ways to improve digital care supporting those admitted for acute sickle cell crisis episodes via emergency hospital admissions and how pain management and episodes can be better managed at home.
