With ever-increasing strains on social care, delayed discharges remain an important issue for trusts. One enabler is good-quality supported housing, offering wraparound care for service users that helps them avoid readmission.

The session will explore how NHS trusts Southern Health and Sussex Partnership are innovating and partnering with the third sector to help service users back into their communities. You will also hear how housing provider Look Ahead and homeless and inclusion health charity Pathway are working in collaboration with the NHS to reduce pressures on inpatient services and free up much-needed bed space.
