This session shares insights from a disease prevalence quality improvement programme delivered at primary care networks across Coventry and Warwickshire ICB. The session will also focus on a collaborative lipid management programme supported by industry, which proactively identifies and reviews patients requiring lipid optimisation for secondary prevention of atherosclerotic cardiovascular disease to prevent further cardiovascular events.
