Admiral Nurse Transitions of Care, Livewell Southwest

Tracy is an Admiral Nurse employed by Livewell Southwest in Plymouth.
She is based at University Hospitals Plymouth and supports individuals and their families living with Dementia within the Acute hospital and through to their discharge destination until they are settled and stable.
She has been working as an Admiral Nurse since November 2022 helping to develop the Transitions of care Admiral Nurse role (Plymouth) with her colleague Emma Hughes (South Hams and West Devon).
Tracy and Emma have received guidance from Rachel Johnson, Regional Account Manager (Southwest) from Dementia UK to develop the service and have recently received an HSJ Partnership award for the Most Effective Contribution to Integrated Health and care. They are supported in their role with Guidance, Teaching and Supervision provided by Dementia UK.
Tracy qualified as a Registered Nurse in 1990 in South Africa having qualified as a General nurse, Psychiatric and Community Nurse as well as a Midwife. Tracy has been working with the Care of the Elderly as a Stroke Specialist nurse, Frailty Specialist Nurse and as a Manager of a Dementia Assessment Mental Health unit before becoming an Admiral Nurse. With this experience she has gained knowledge and contacts that help her to provide care, advice and support to individuals living with Dementia
