Working across a large and diverse PCN in the North of Sheffield, I lead a team of Care Coordinators to support a variety of patients through their primary care journey. Ranging from providing wrap around support for high intensity users to ensuring people with cancer access the care they need in a timely manner. I am passionate about reducing health inequalities and providing an holistic, person centred, health service especially for people with learning disabilities from diverse backgrounds
