Anna is a consultant community geriatrician leading the community frailty team in Hull and East Riding, and providing strategic clinical leadership with regards care homes to the system. The multidisciplinary, multi provider team delivers proactive comprehensive geriatric assessment for those living with frailty in their own homes in the bespoke Jean Bishop Integrated Care Centre, as well as in-reaching into care homes. The centre is located in an area of Hull with high levels of health inequalities and deprivation.
With colleagues she has led the development of the reactive frailty response through urgent community response and frailty virtual wards. Through both the proactive and reactive frailty programmes those living with frailty in Hull and East Riding have seen improved patient reported outcomes, as well as system benefits relating to sustained reductions in unnecessary ambulance dispatches / conveyances, and emergency attendances / admissions.
