CHASE × Medacy is a directly commissioned NHS delivery partner, working with ICBs and PCNs where internal capacity is constrained. We combine proven delivery models with NHS governance and clinical oversight to reduce risk, workload and delivery pressure across the system.
With over 1.5 million NHS patient contacts and active programmes across multiple ICBs, our work is operational and evidenced, not theoretical. Our programmes are designed to be replicated, with governance frameworks and clinical oversight localised to your population and priorities.
We focus on delivering care closer to home, shifting activity out of hospital and supporting population health at scale — in line with the strategic direction ICBs are required to pursue. All services sit within NHS governance frameworks, with clear clinical accountability, information governance and NHS data integration built in.

CHASE × Medacy presents delivery models already operational in the NHS, designed to address immediate and strategic commissioning gaps:
- Population health and prevention: Large-scale call and recall delivery for childhood and adult vaccination, cervical screening and preventive outreach. Multilingual, multi-channel outreach including the NHS App, text and telephone, designed to reach under-served populations and support QOF delivery.
- Chronic disease management: Medicines optimisation, diabetes and weight management reviews, DMARD and biologics pathways, delivered by experienced clinical pharmacists. Designed to address capacity lost to system pressures.
