CareMonitor
CareMonitor is an award-winning digital healthcare platform that supports care delivered outside the hospital.
Our platform supports virtual wards, hospital at home services, remote patient monitoring and community-based care. It also supports longer-term condition management and post-discharge follow-up.
CareMonitor is designed to be flexible. Services can configure the platform to reflect their clinical pathways, team structures and ways of working, rather than adopting a fixed model.
Clinical and operational teams use CareMonitor to manage patient cohorts, coordinate care and maintain visibility across services. It brings monitoring, tasks and escalation into one place, supporting day-to-day delivery across acute, community and virtual settings.
The platform is designed to sit alongside existing systems, with interoperability and governance built in.
