Hear from Deb Lowe, NHS England and NHS Improvement’s National Clinical Director for stroke care and Laura Rogers, Senior Stroke Manager about national plans to improve stroke rehabilitation in England.

Publication of the National Service Model for an Integrated Community Stroke Service (ICSS) has ensured Integrated Stroke Delivery Networks are guided by evidence-based recommendations. This describes a model for delivery of home-based stroke rehabilitation by specialist community based multidisciplinary teams, combined with integrated and collaborative working with social care and the voluntary sector to ensure stroke survivors and their families needs are met.

The session will also showcase the National Rehabilitation Pilot project, in which sites are trialling delivery of the ICSS model incorporating increased availability of seven-day home-based therapy, virtual rehabilitation, structured follow up in the community, improved psychological support, and delivering vocational stroke rehabilitation.
