Starting with the patient: redesigning care pathways across primary, community and acute care

Over recent years, and as a result of the NHS Long Term Plan (LTP), there has been an increasing focus on ensuring patients are at the heart of care pathways. Providers are now working together to redesign pathways – and their operations – so that patients receive the right care, in the right place and at the right time.

This session will provide some examples of recent pathway redesign that have improved patient outcomes and alleviated workforce pressures. We will highlight how patients were part of the process and the value of primary care leadership, and will explore the following:

  • What ignited the decision to redesign the pathway?
  • What enabled cross-system integration to happen?
  • What has been the result of the pathway redesign?
