People-powered partnerships: how to improve lives and achieve a 25 per cent reduction in secondary care costs

The session will focus on community led health partnerships and projects in North East Lincolnshire (Thrive.NEL) and Newcastle (Ways to Wellness), which achieved 25 per cent reductions in secondary care costs. This outcomes approach is now being extended in local areas due to its success.
