Population health management: how primary care is overcoming barriers to implementation
This panel session focuses on the practical implementation of population health management (PHM) approaches in communities across the UK, with a particular emphasis on how we can transform our digital and data capabilities to make PHM a reality.
Discussions will be centred on digital infrastructure requirements and addressing data and digital barriers, such as data quality importance, data security, collecting the right types of data, best practice methods of data storage, analytical capabilities, and risk stratification. Set against a backdrop of health inequalities, discussions will be contextualised by relevant case studies that are contributed by the expert panel.
