Population health management: from integrated data to impacting population health outcomes and reducing inequalities

One of the driving forces behind integrated care systems is the opportunity to improve population health outcomes. Using a population health management (PHM) approach can reduce inequalities but requires a systems approach, enabling health and care professionals to work collaboratively through identifying population need and joining up the right person with the right care.

This session will explore what changes are required within systems by examining the enablers to PHM and how they have impacted health outcomes.
