We know people recover quicker at home and for many, that is where they would rather be. So what more can we do to deliver healthcare at home?

This session will explore the transformative enablers for providing quality care closer to home (looking at systems, providers and people), as well as the barriers that may be affecting care and how they can be overcome. The session will examine short-term and long-term change that could enhance community care.
