From practice to place to system: what is an integrated neighbourhood team?
The Fuller stocktake report set out the need for integrated neighbourhood teams as a next step to integrating primary care, evolving from primary care networks (PCNs) and creating ‘teams of teams’ to embed a sense of shared ownership of action for improving the health and wellbeing of the population. But what does this look like in practice and, crucially, how can practices, PCNs and systems help one another to make integrated neighbourhood teams a reality?
In this session, we will hear from those at different stages of the journey to becoming integrated neighbourhood teams and how they have approached the challenge through the lenses of practice, place and system.
