How well do we understand older people’s experience of increasing frailty and are we equipped to support them to live their most fulfilling lives? This session showcases examples of teams collaborating across organisations to better meet the needs of older people in a personalised way: stepping away from seeing older people as their health conditions and focusing on their unique lived experience to make sure they are valued.

Speakers from primary care, adult social care, voluntary sector, and community services will share their views on addressing the impact of frailty across all communities through adaptive approaches.
