Universal personalised care: Implementing the Comprehensive Model is the action plan for the roll out of personalised care across England. It sets out how we will increase the number of people benefiting so they have more choice and control over their mental and physical health just as they have control over other aspects of their life. The NHS has a specific target for the number of people who could benefit from personalised care by 2023/24 which is outlined in the NHS Long Term Plan. People benefit from a personalised care approach by using a number of interventions including personal health budgets, personalised care and support plans, social prescribing, shared decision conversations and supported self management

In this session you will hear how the NHS provides people with the opportunity to have more choice and control over their long term conditions through personalised care and NHS @home approaches.
