This session will look at how AHSN North East and North Cumbria facilitated a system-wide approach to affect behavioral change, embed historical learnings, mobilise resourcing and maximise funding acquisition in order to address health inequality in lipids management.

We will demonstrate learning from this case study, including:

• how to seek and capitalise on opportunities to address health inequalities
• identification and mobilisation of clinical champions across the system to act as agents of change and champion action
• the importance and benefit of primary and secondary care integration in improving shared responsibility, improved awareness and improved workforce capability
• examples of capitalisation of resource opportunities: personnel and process.
