Driving national impact through local insight: tackling health inequalities and improving care for excluded communities
Life expectancy in the UK has been stalled since 2010 with our nation’s health following a social gradient where people residing in more deprived areas live shorter lives with more years spent in poorer health.
This session will showcase The AHSN Network's efforts to tackle health inequalities at a national and local level, working alongside integrated care systems to harness local insights that can accelerate the adoption, spread and accessibility of products and services, achieving a more equitable health and care services for everyone. The session will explore the importance of effective community engagement and culturally relevant information, giving specific examples on the steps taken to identify and overcome barriers that prevent some ethnic minority groups from accessing care needed to manage asthma effectively. You will also hear about an innovation supported by the NHS Innovation Accelerator that is enabling clinicians at the point of prescribing, dispensing or discharge to print instructions, warnings and information in accessible formats. Finally, learn about how the national rollout of UCL Partner’s Proactive Care Framework is supporting the primary care workforce to offer long-term condition management to people with the greatest need.
