How can we accelerate and strengthen collaboration across health and social care to help people to manage their health in a way that is safe and supported, and in their own home?

This session will explore how multi-disciplinary teams from health, social care and the voluntary, community, faith and social enterprise sector are working together to realise the ambition to boost out of hospital care and provide more support to people closer to home.

This is an opportunity for speakers to share and discuss examples of community care models that are shifting the approach from reactive to proactive care and preventing unnecessary admissions to hospitals and residential care. These models of care include Enhanced Health in Care Homes, Urgent Community Response and Anticipatory Care.
