Collaborative working to increase uptake of annual health checks and vaccinations for harder-to-engage groups on the learning disability register

The session will address ways of overcoming barriers and challenges to increasing uptake of annual health checks for harder-to-engage groups on the learning disability register.

It will be based on the experiences of NHS South Yorkshire Integrated Care Board, Sheffield’s Foundry Primary Care Network, and Sheffield Mencap and Gateway, who worked collaboratively to provide targeted outreach to support and empower people with a learning disability to get their annual health check, to understand their health action plan, and to access vaccinations.
