Neighbourhood health: what should look and feel different for patients?

How can and should a neighbourhood health service improve patients' outcomes and experience of healthcare?

As increasing numbers of people live for longer in ill health, and often living with multiple health conditions, neighbourhood health services have the opportunity to transform the way care is delivered for and with patients. But how will it actually look and feel different for them?

Duleep Allirajah will explore The Richmond Group of Charities' patient-focused research about what improved care really looks like, while Jane Taylor will talk about her experience of fragmented care for multiple health conditions, and what would make a positive difference.

Kate Petts will then discuss the learnings from the Complex Long-Term Conditions Service being piloted in North Central London Integrated Care Board, including how it involves a lived experience advisory group. Caroline Taylor will explore her belief in the neighbourhood care model, described originally by the National Association of Primary Care, and experience of delivering differently in Calderdale.

The panel will discuss how those implementing neighbourhood health can remain focused on what will look and feel different for patients, as well as staff, structures and processes.
