Moving safely from hospital to home: developing a primary care medicines discharge service

The Sunderland Medicines Discharge Service is provided via a hub of pharmacists and pharmacy technicians funded by the Additional Roles Reimbursement Scheme. It processes discharge documentation within 48 hours of care being moved from hospital to home.

This session will explore how the model went from an initial concept to its current delivery. It will look at how local primary care networks piloted a new way of working, aiming to provide safety, efficiency and reducing the pressure on existing GP admin. It will explore data and case studies from the past two years demonstrating good practice and exploring the challenges. Plans for 2025/26 will be shared, including the evolution of a prescription management service.
