Dementia UK is the only dementia nursing charity in the UK. Our Admiral Nurses (dementia specialist nurses) work across all health and social care sectors. We have developed a new model called ‘transitions of care’ whereby an Admiral Nurse service tackles one of the most challenging health and care system problems – the care of person with dementia with complex needs in hospital, their discharge from hospital to a care home or back home, and the prevention of a readmission. The service intensively case manages families affected by dementia during this time, bringing together the health and social care system, placing families at the heart of decisions and outcomes.

This session will explore the model of Admiral Nursing and the impact it can have.
