This panel session will explore the transformative potential of technology and data in enhancing integrated neighbourhood working, with a focus on bringing care closer to home.

Experts from across the system will discuss innovative approaches and share insights on how digital tools and data analytics can improve community health outcomes, streamline care delivery closer to home, accelerate efficiency gains and foster collaboration among healthcare providers.

The session will highlight the importance and practicalities of working with the wider system to provide targeted and appropriate services locally. And how using technology and data will enable newly formed integrated neighbourhood teams to make data-driven decisions when adapting and creating pathways to bring care back into the community.
