Expectations vs actions: the AAR approach to patient safety and continuous improvement

11 Jun 2025
Exchange 8-10
Mobilising change / improvement , Quality and patient safety

After Action Review (AAR) features in the new Patient Safety Incident Response Framework (PSIRF) as one of the key learning tools. This interactive session will give participants an opportunity to consider how AAR uses past experience to inform future action.

The first part of the session will introduce the tool of AAR and the key concepts underpinning it, through a series of group activities to demonstrate the value of using AAR as a tool for continual learning. We will explore the experience of individuals’ expectation versus personal and collective action and how the difference between the two offers us the space to learn and do differently in the future.

The second part will bring together a group of system leaders implementing AAR in their organisations, offering reflections from their experience of doing AARs and their impact so far.
