Driving system-led improvements to patient-centred care for long-term conditions
Long-term conditions (LTCs) currently have a vast impact on patients, the health system and the economy, accounting for £7 in every £10 healthcare expenditure, 50 per cent of all GP appointments and 70 per cent of inpatient bed days.
In recent years, improving the management of LTCs and multiple LTCs has risen up the national policy agenda. And improving the outcomes and care of patients with LTCs will be central to the three shifts in the government’s ten-year health plan: moving care from hospital to community, treatment to prevention and analogue to digital.
This non-promotional panel, organised and funded by AbbVie, will discuss the changing landscape both nationally and locally for improving care for people with LTCs. This will include how new patient-centred metrics can be developed and used to assess and track patient outcomes, how more innovative care models can be deployed and how new technologies can be used to improve patient self-management.
The panel will include a discussion of a new project led by National Voices and Future Health Research* outlining policy opportunities for improving care for people with long-term conditions.
