Partners in City and Hackney have made neighbourhoods the default way of working for engaging with communities and delivering out-of-hospital services.

Join this session to learn:
• how the approach has evolved since 2018, the challenges faced, and how the trust spread momentum for the change
• how City and Hackney has used neighbourhoods to engage with local communities, address health inequalities, find out what matters to them and work together to find solutions.
• how community health and social care services have been re-organised around the neighbourhood footprints, the challenges of doing this and the benefit this has brought to patients and staff through closer working between teams and teams feeling more connected to their neighbourhoods.
