Creating conditions for change: transforming outcomes through collaborative care

This session will demonstrate the productivity and efficiency gains realised through clinicians working closer together across primary and secondary care.

Using the example of a health inequalities improvement programme delivered in Redbridge, north-east London, delegates will hear how resources were maximised to benefit both patients and the system through the upskilling of primary care teams; use of technology; and collaborative working across primary and secondary care.

Managing more complex patient needs through this collaborative approach reduces the demand for reactive and emergency care, improving outcomes for all patients living with cardiovascular-related disease(s), while addressing inequality of access, experience and outcomes.
