Neighbourhoods are critical to local people and the equity of access and outcomes they should expect. This session is led by leaders from acute, primary care, faith sector and our neighbourhoods and demonstrates the work within one of the most progressive and impactful place-based partnerships in England (hosted and driven by our local acute trust).

With a spotlight on Peterborough, serving a diverse population with significant health inequalities, you’ll learn how we are tackling these challenges head on. By embracing our award-winning population health management approach, we are delivering care that works for local communities and the most vulnerable.
