Bridging the demand and need gap: is population health management the answer?

We know all too well that gaps in access to care are widespread, and that we experience an ever-expanding gap in our health system’s capacity to meet the demands of our patients.
Could population health management (PHM) be part of the solution? Join a case-based panel discussion on how population health management can unlock capacity, reduce health inequalities, and improve system resilience. Our conversation will focus on how PHM helps make the most of limited resources and how NHS systems can take a leadership role in scaling PHM tools that work at the frontlines, getting innovations to a point of implementation within Integrated Care Boards.
Colleagues in South West London and North East and North Cumbria will share real-world examples of how they leverage effective risk stratification to reshape care delivery through the ProActive Register Management (PARM) Tool for Diabetes and the PARM Tool for Obesity, which are Lilly-donated cardiometabolic health tools. We will also hear about Derbyshire’s approach to local pathway development for weight management.
